Required vaccines
The below vaccines are required to enter school (K-12):
- Diphtheria, tetanus, pertussis (DTap)
- Hepatitis B
- Inactivated poliovirus (IVP)
- Measles, mumps, rubella (MMR)
- Tetanus, diphtheria, pertussis (Tdap)
- Varicella (chicken pox)
Notes:

Students 4 through 6 years of age are required to have their final doses of DTaP, IPV, MMR and Varicella prior to kindergarten entry.
- Students are required to have Tdap prior to entry into sixth grade. One dose of Tdap is required for sixth through 12th grades.

Troubleshooting
To successfully move through the state’s online immunization education module and obtain an exemption certificate, it is recommended that you review the Technical Guidance and Troubleshooting Document.
- We do not recommend using a mobile device to complete the course and form.
- The module is available 24/7. It can take approximately 20 minutes to complete.
- Important: Non-medical exemption certificates expire each year on June 30. If you submit a non-medical exemption certificate on or before June 30, it will not be valid for the upcoming school year unless submitted during early registration.
Drug Intervention Services
Our mission is to support students and their families experiencing substance challenges. Our school nurse is trained to discuss and support students with substance use and misuse. The school nurse offers screening and guidance for any student who has been suspended for substance use, who is suspected of substance use, and referred by school personnel, parent or guardian, or self-referral. We offer support to students and their families to help navigate resources and services that may be available.

STRIDE Community Health Center
STRIDE Community Health Center is a Federally Qualified Health Center (FQHC), providing high-quality health care to all members of the Denver-metro community. Although we are not a free clinic, we work tirelessly to ensure that all our patients receive the care they need, regardless of ability to pay. STRIDE uses a sliding fee scale based on family size and income only.
Confidential health services
Did you know that adolescents 12 to 17 years old can receive confidential mental health and reproductive health services in the state of Colorado?
STRIDE wants to ensure that our younger community members receive the care they need to support a healthy and balanced lifestyle. The appointments are free, can be done in person or over telehealth, and do not need a guardian’s permission to attend or participate.
Health Services in Jeffco
Jeffco Public Schools' Department of Health Services provides health consultation and resources to support students and their families.
Each school has a district Registered Nurse (RN) who consults with students, families, school staff and administration. Ask your student’s school for their district RN contact details and notify the RN if your student has a health need that requires support during the school day.
Each school also has a school health aide who does day-to-day care of students such as medication, Band-Aids, and ice.
Keep your student’s health concerns updated at registration and as conditions change during the school year. It is important for Jeffco to have current contact information of families.
When Students and Staff Should Stay Home
Jeffco follows the recommendations from Colorado Department of Public Health and Environment (CDPHE) on when students and staff should stay home from school, work or childcare. District RNs provide guidance to school administration following the How Sick is Too Sick document.
The best ways to stop the spread of infection is through good hand washing and staying home when sick.
Immunizations
Colorado law requires all students attending Colorado schools and licensed childcare centers to be vaccinated against certain diseases unless a medical or non-medical exemption is filed.

Medical Forms
Jeffco families must submit a medical provider-signed agreement if their student requires medication during the school day.
Mental Health Supports
(SCHOOL NAME) promotes a safe, positive learning environment by supporting mental health.
Jeffco Public Schools recognizes the growing concern with injuries resulting in concussion or concussion-like symptoms. Mild Traumatic Brain Injury (TBI), or concussion, in children is a rapidly growing public health concern.
Jeffco Public Schools partners with Hazel Health, Inc. to provide free, additional physical and mental health care services for students. This a telehealth extension of support to Jeffco students enrolled at district-managed schools.
Jeffco Public Schools' Certified Application Assistance Site (CAAS) operates through the Jeffco Public Schools Community and Family Connections (CFC) program. CFC assists any individuals inquiring about Health First Colorado (Medicaid), Child Health Plan Plus (CHP+) and other health-related resources.
Hearing and vision screenings are conducted for students in their schools according to the Colorado Department of Education guidelines.
The Department of Health Services at Jeffco Public Schools recognizes that any student or family can have lice. Jeffco’s district nurses follow several protocols and are an excellent resource for school administration, teachers, parents and families.
Parents and guardians of Jeffco Public Schools students can purchase student accident, athletic and dental insurance coverage.
